Haven Hospice provides compassionate end-of-life care in 18 North Florida counties, including Duval, Clay, Nassau and St. Johns. Their services include hospice care, palliative care, dementia care, advance care planning, and bereavement support. Care is delivered by an interdisciplinary team in a patient’s home or wherever they call home, including assisted living facilities and nursing facilities.
